Custom online form submission error

I have built a custom HTML online form using <NLTAGS> and all seems to work fine - it should create a new lead and alert staff to contact the enquirer.

One we created using Netsuite standard layout works fine but with this custom code one when we hit the submit button we get

Page Not Found

https://forms.netsuite.com/app/site/crm/default.aspx
Click the 'Back' button on your browser to go back.


The data does not get uploaded so the submission clearly fails - having checked the form against the ones which work the only difference is the use of an external template.
