<NLCREDITCARD> changes?

Where do I go to change the layout of the Credit Card fields on the Remittance Form?

It is called in the HTML layout as <NLCREDITCARD> which gives me a horrible layout of credit card type, number, expiration date and signature. I need to improve the spacing of these elements and add a "name on card" line.
