Web order required fields not making it to Sales Order

We put a custom required field on our web store orders. A custom Customer PO#.  It seems that occasionally this does not make it to the sales order.  During in house test we have seen this not go thru even though it was put on the order.  You can't save the order without it.

Has anyone else seen this?
