Lead Qualifier Saved Search

Can anyone help me with the function formulas needed to accomplish the following results?

I need to create a saved search that displays the milestones (and the date the milestone was reached and the performer) from Lead to Customer.  I know I need to reference the System Notes fields:  SET BY, OLD VALUE, and NEW VALUE but I'm having a hard time putting it all together. 

Here are the search requirements:
1) Two (employee) Leads Qualifiers will be changing lead status from Lead-Unqualified to Lead-Qualified.  
2) Leads Qualifiers + Sales Reps will be changing Lead-Qualified to Prospect-In Negotiation to Customer-Closed Won, etc.
3) Show the date converted, Lead name, city, state, and sales rep.
