Sync causes duplicate events

I've got a user that has Outlook 2003.  They use Intelisync to sync outlook to Netsuite and Activesync to sync Outlook to their mobile phone.

Entering an event into Netsuite and then syncing to Outlook and then syncing outlook to the phone causes duplicate or triplicate events.  The events have exactly the same names and times.

Anyone else have this issue?
