Bulk Fax Frustration

I've been struggling with setting up a bulk fax and am wondering if anyone else has ever had any success with the feature (or if I'm wasting my time).  Our channel customers live and die by the fax machine so this is very important to us (I know--old technology, but the fax response rate is about 1000X better with these folks than alternatives).

The help files say you can create a fax template from PDF: I can not get NS to recognize any PDF templates we upload (we can do it with letter and e-mail templates, just not faxes).  So, I tried creating a bulk fax from HTML (using nothing fancier than HTML tables) and the output gets screwed up (changes font size, color, background, spacing which is incredible--I haven't even gotten to adding our logo image).  On top of that the test fax includes a broken cover page (even though we have cover pages turned off everywhere I can find the option).
