create checkboxes group

Hi all,
How do I creat checkboxes group which similar to the following?

Business Type [ ] Retailer
                    [ ] Service Company
                    [ ] Trading Company
                    [ ] Other

Those checkboxes will be placed in the customer form?
I can only create one checkbox.
