Customizing NS Checkout?

I've been tasked with improving the usability of our checkout pages by customizing the NS cart. But I've also been told that isn't really possible.

Mostly I'd like to just lower the amount of arbitrary alignments going on and favor a vertical flow, left to right.

What are people's experience doing this? Are there painless third party options that integrate? I feel like I have to ask this every day and the answer is never "yes."
