Relationships not showing on Lead/Prospect/Customer Records

I've tried everything, when I edit the Entry Form the Relationships Sub Tab is checked to show, but for whatever reason it just doesn't want to show up.  The Custom Entry Form is set to Form is Preferred and the Inactive box is not checked.

The Relationships show on the Contacts, but not on anything else...

I've also found that when I move fields in the Entry Form it does not move them on existing records, it just makes the changes for new records.  Am I doing something wrong?

Please help!
