Free Shipping BUG is costing us money

We came in today and when processing orders from over the weekend we found three orders with a shipping cost of $0.

Anyone else finding this?

No coupon code. No correlation in where they're shipping to or what they ordered. They are all UPS ground, but 95% of our orders are UPS ground.

This is costing us money because we're honoring the free shipping that they received.

Has anyone else had this happen?
