Sporadic and Infrequent Database Read, Write and Simple Saved Search Performance Issues

I am writing to see if anyone is experiencing general performance concerns in their accounts (not eCommerce related).  We are seeing the following issues in multiple client accounts that appear to be related to bad index issues.  These issues are sporadic in that most of the time, on average, performance is reasonable.  Yet, invariably, there are performance issues that crop up that are infrequent and frustrating enough to wonder "what is going on?". Here are some of the issues.

1. Loading and or saving a Sales Order may take 30 to 90 seconds
2. A Netsuite Advanced Project has references to saved searches to draw related invoices.  At times, it can take 30 to 90 seconds to load the records.  There is only 5 items in the return list.  The project is referenced in the invoice name transaction column, right where you expect to have an index.
