Item Labels - Avery 5160 Format?

Has anyone been able to format a Transaction Form PDF layout, type=Item Labels, to match Avery 5160 size labels.  We print Item Name/Number, Display Name, Color (custom), Size (custom), and UPC# as a bar code. 

NetSuite mentions in the 'Help' that the 'standard mailing label layout' matches Avery 5160 and 5260.  I copied those settings (Height, Width & Margins) into my custom item label PDF Layout, yet it still does not format correctly.  Top margin is too large, and the labels are not long enough. 

Has anyone been able to get this to work?
