Navigation Bar Showing up 5 times...?!

I've created a custom navigation bar that I want use, and made it live on our site for that matter, but it's showing up 5 times in a row.  I'm guessing it has something to do with the fact that I have 5 tabs and it's inserting it once for each tab....?  I've experimented with the 'website theme' section, adding this here and taking that out there, but haven't found a solution.

My goal is to have the new nav bar show up once along the left side.  Any ideas?
