1096 Form - Not lining Up

Hello,

It looks like printing the 1096 form from NetSuite is not lining up correctly.

I just wanted to check to see if anyone else is experiencing this, or if it is just isolated with us.

It looks like the 1096 form has been changed for this year, and I'm not sure if NetSuite updated the change with the report.
