Error when creating sales order in customer center

Hi,

I have been getting an error message " An unexpected error has occurred. Please go back and try again." when submitting a sales order in customer center.
Not sure if I am missing any settings but I have checked everything and it's all OK I think.

Any help is highly appreciated.
