Reconciliation Report header incomplete

The printed reconciliation report has significantly changed in this version.

A glaring bug is that the report doesn't have the name of the reconciled account in the header. Nor is the final reconcilation date prominant. (The irrelevant date of the last reconcilation is more prominant).
