Memorized Transactions going to Unapproved Payments?

We have over 1000 Memorized Transactions for our "AutoPay" feature which we use for monthly service subscriptions. They are Cash Sales.
Apparently, sometimes the Memorized Transactions don't work correctly. On the scheduled day, instead of the Credit Card being processed and going into our bank account, or being rejected, the Cash Sale is being recorded as an "Unapproved Payment". Until a couple weeks ago, I did not even know there was such an account. And worse, that we had been missing thousands of dollars in payments.
This is NOT an online or outside process. These are supposed to be charged to the customer credit card on file. The card is not Expired, nor is it being reported as Declined.
