Blocking Fraudulent Account Creation

How does everyone handle fraudulent account creation?

We have someone running a bot against our website, creating fake customers accounts and trying transactions against a huge list of credit cards. That ends up maxing out the transaction limit at our processor and blocking purchases from real customers.

We have disabled guest accounts, added Captcha to account creation - but to no effect.

Solupay say they have seen a big increase in Netsuite users getting hit with this type of attack.

Ideas ?
