Thank you both for your answers, both are relevant to our operation. In this case, editing the customized pdf/html form was what I was looking for. The print screens to follow the steps was very helpful.

It was added as a "message" in the communication tab and the "to be printed" box was checked. I am thinking it might be something with the form design? Not sure, very new to NetSuite.
