We've almost got it. The report is now working however, I would really like to add the created date and the created by fields to the results. I've added them to the results tab however, they don't appear on the final search.

Thanks Jack. I've added to the source code per your instructions and it is working however, I now have 2 columns showing the MPN. Can you guide me through deleting the unnecessary MPN# column? Here is a screenshot of my source code.

Continuing with this form, I have run into another issue. The MPN # is not showing up on the PDF form of the PO. The MPN fields on the new element function are grayed out and I am unable to select them. I'm not sure why this is the case. What am I missing? My apologies for not replying earlier. I was out of town for a holiday weekend. This is the field ID info. It does appear that we have renamed the field from department to division.
