I found the answer (for me anyway): it happens due to blocking third party cookies. I use Chrome so I had to add the following to sites that can always use cookies. The last one might be redundant, but it's working now.

The client says that it is now working to create a sales order using the matrix subitem without the fully-qualified name including the parent (PARENT : CHILD).
