Ask a question thru a workflow?

Can you ask a question in a workflow and then store the answer on the record that initiated the question?  Basically at save, if a field is not marked yes or no, I want to ask the question if it should be yes. If they respond it should be yes, i want to update that field with yes.

I attempted to use the CONFIRM action but could not get it to execute.
