Has anyone created affiliations (with relationships or not)?

I am creating affiliation records for customers that are members of my organization. For instance, John Doe has a "Sustainer" membership based on his contribution, at his level he receives specific benefits these benefits are also available for 1 person he names as well as up to 2 minors that he designates. The way I am trying to accomplish this is by creating a Custom Record Type. This will be essentially how it will look on the customer record, for instance this would be on John Doe's account:
