New Fields in Work Order Completion form

Hello Experts,

Can you please help and guide us on how can we create a new field in our work order completion form.

We are a manufacturing company and would like to add some additional details in our work order completion form.

We like to have field such as Operator's name, Shift timing (AM/PM), machine list (Dropdown list as 1, 2,3 4 and so on)

Please help us!!
