Inventory Transfer - Pulled Items From Location, Did Not Add

A user performed a basic Inventory Transfer yesterday.

The inventory was pulled from the original location, but did not add to the new location. Both locations have had inventory transferred to and from them before and this item has a lot of transactions associated with it.

There is no GL impact data at all. I have checked previous transactions and they look fine.

Any idea what's going on?
