Credit Memo / RMA Reason - Mandatory

Good afternoon guys! We are looking to have a field added to the RMA and credit memo that will be mandatory. We want it to be called "Reason for Credit/Return" and we want it to be carried over from one form to the next. Who can help me make this happen?! Thanks so much in advance!
