How to solve a Case When Formula not working as expected?

This seems very straightforward, but it isn't working for me.

I am using the following to exclude records in criteria.

The formula should evaluate to zero for the record below as all 4 statements are True. However, this snip from the results indicates otherwise.

Ideas?
