Why does ID Already Exists even though I'm not mapping any IDs

I'm not sure what this error is talking about when my CSV doesn't contain any columns for Vendor ID, External ID nor Internal ID. I'm also not mapping any of them.

What ID? It might be talking about the Map name, but I'm doing a 'Save As' and making up completely random names.
