Is possible to show a list of all users role assignments made and who did them?

Does anyone knows if possible to show a list of all users role assignments made in FY24 and the user that did them?

I was able to create an employee saved search showing role change, role change action and role change date fields, but i cannot find the user who made the change

Would need a Report/SS to see the Roles assinged and unassigned to each Employee

This would be the Access tab > History subtab of the Employee record

One of our ACS customer is requesting this information due to audit purposes.

We had tried with SS/Workbooks and there is no way to bring that information.
