Suitelet shows "You do not have privileges to view this page"

Hi,
On our Sandbox 1 server, we get the error "You do not have privileges to view this page" when using the public URL.

The script has the "Available without login" checked, which was working and still working on production; only the sandbox server has the problem.
It just started a few days ago out of nowhere.

I have reployed the script, checked/unchecked the "Available without login", chosen all roles, etc..

Is there something I can check/do that will help fix or track down what is causing this?
