PO Workflow randomly hit error when remove Close Button

We have a Workflow setup on PO approval process, while we notice the WF hit error randomly when execute the remove button step, and it cause the WF failed execution and PO approval status stuck. Please advise if this is Netsuite backend error ? how to address it.
