Need help with default value formula

I don't have any experience using functions in Netsuite, but I need to create a code for new customer accounts. Can anyone tell me how to set the default value of a custom field to equal the customer's initials plus the last four digits of their phone number?

Example: John Doe, phone number 555-555-1212
field should populate: JD1212
