Shipping charges not being calculated

We're experiencing a v. 11 problem with creating sales orders. Things are fine on the web site, but when a customer calls, the shipping charge is not being calculated. It defaults to the proper item (flat rate) but lists it as $0.00 rather than $5.95. If I switch to another shipping item and then switch back, it puts in the correct number.

Any thoughts?
