Custom Transaction Columns Not Printing

We have 3 customized columns in our transaction forms. For some reason the data is not priniting.

The custom columns were set as a Free-Form Text and the columns are marked under the "printing field" tab. They have been working previously and stopped working. We also noticed that the "display name" is not either printing.
