PDF Form Printing askew

When I print a PDF sales order the top margin on the right side of the page is approximately 1/8 inch smaller than the left side, making everything slope slightly up and to the right.  While "up and to the right" is a great mantra for stock analysts I really don't care for it on my customer forms.  I've mined the past posts and the only thing close concerns printing shipping labels.  I'm using Adobe Reader 9 and notice that NS is generating content based on Acrobat 4.x.
