NetSuite generating duplicates

I'm not sure if this is a bug or what, but seems on rare occasions NetSuite is making 2+ copies of the same Lead simultaneously; In 3 days I've found 5 instances of emails with 3+ duplicates.

Worst case I've found today was one record with 7 duplicates created by the same agent on the same second.

I've checked workflows, and they are both field-changers.

Anyone else noticed something like this?
