Duplicate SO's Created via Web Store

Some SO's placed via our site builder web stores have been getting duplicated somehow.  The first duplicate dates back to February of 2014 but they started picking up in April (seemingly shortly after our upgrade to 2014.1).  We've had a total of 38 duplicates since the issue started.

[*]1 in Feb
[*]3 in April
[*]10 in May
[*]5 in June
[*]8 in July
[*]11 so far in Aug
[/LIST] I created Case 2000855 with NS (still no defect filed though).  The support rep told me NS was aware that this was an issue with Reference Checkout but not with Site Builder. 

Anyone else seeing this?
