Inventory Quantity Committed Issues?

We've been having inventory quantity count issues for the past year.  We've filed numerous cases and in turn have had numerous defects created and "fixed,"  however, the issues persist.  I'd really like to know if we're the only netsuite users having these types of issues, as they are critical for our business, yet Netsuite doesn't seem to have any answers for us. 

The issues we are facing are:
Items not getting committed on an order. This is pretty self explanatory.  Once an order is approved, all items that have the available quantity should automatically get committed.  This wasn't happening on some orders, so our quantity available wasn't correct.
