Beta site double charging credit cards?

Can it be? HOW can it be?

We use some custom scripts that automatically charge customers when a fulfillment is created. Yesterday from about 11:30am to 1pm, we had about 30 customers double-charged for orders that were fulfilled on Monday. (We do use some custom integration code and a non-standard credit card processor.)

This set off a massive scramble to audit our code and review log files. We could not find anything that could have possibly caused customers to have been re-charged for last Monday's orders. I began to suspect one of my new employees was doing something improper. We interviewed him, and came to believe that he was not running any transactions that could have billed a credit card.
