Surcharge for shipping to Canada

How would I go about setting up shipping charges based on Shipping address?  I have 3, 4 shipping methods setup based on the order subtotal. 

The only international destination we ship to is Canada, but we have no special shipping method setup for it.  I'm wondering what options I have if I want to setup some type of surcharge system in place where an order to Canada would be charged an additional $5 on all shipping methods.

I can also setup another set of tables for shipping to canada, but is there any way that I can limit canadian customers to only those particular choices?
