Issue with Shipping Packages?

Today, we tried experimenting with predefined shipping packages.  I setup 3 shipping packages and assigned these to 4 different items.  We also checked the "Ships Individually" box on these 4 items.  We did nothing to any other items and none of the shipping packages were designated as a default.
When we tried to fulfill our first order for the day we ran into issues.  The order was not for any of the items changed.  But the fulfillment page (packages tab) automatically had a package added at the bottom of the screen (as opposed to waiting for information in the fields, basically as if the Add button had been clicked).  In and of itself, that would just be a little confusing, not a big deal.  But then when we look at that added package it has had the package size predefined as being 12" x 12" x 3".  That is
