PageInit Error in IE and Safari

Not sure if this is the correct section to ask but  I am getting an error that reads:
An unexpected error occurred in a script running in this page.
PageInit(pageInit)
customform

JS_EXCEPTION
ReferenceError 'PageInit' is undefined

When I am in Windows IE I get the above error. When folks are in Mac Safari, they get the same message as well.
I have checked and triple checked. The js file source is being referenced. What am I missing?
