HTML Online Customer Form Template

When you create your "own" HTML Online Customer Form Template: Where would you place the <NS> tags? I know, which ones I have to use, but am lost in where to put them.

For example:
<TD><INPUT type="text" name="company" id="CompanyName" maxlength="300" tabindex="3"></TD>
Where would you place the <NSCOMPANYNAME> tag?
