Customizing Gift Certificates

Does anyone know of a way to customize the website template for Gift Certificates?

I'm trying to apply some additional text explanations, and maybe even a new email address form field, as well as modify the text for the "Recipient Email" field. We want to better direct "gift" certificates - when they're being bought as a surprise. Kinda ruins the surprise when the recipient gets emailed.

I tried applying some item options to the gift certificate, and they finally showed up, but it's clunky.

I was considering creating my own custom template for it or using Javascript to modify the form onload.
