Invalid taxitem reference key -7 When item added to cart

Hi All,

Anyone come across this? Invalid taxitem reference key -7.  When adding an item to the cart?  It only happens if you are not logged in and it does add an "item" to the cart but you can't remove it. 

Here are some background stats
[LIST]
[*]We have 4 states set up to be taxed, and taxes are set to "user" not ALL or NONE.
[*]The second time you try to add the item to the cart it works fine (but you still have an item you can't remove stuck in the cart)
[*]When the error happens with a matrix item, the matrix options that were selected to not get added to the item that is in the cart. (on the second try the options do come over)
