Odd Web rendering since upgrade

We have a custom ecommerce interface, and everything was as intended for two years.  But since the update we're seeing some odd behavior on a single page.

This one info item page with a simple form on it is displaying the green block of text.  And I cannot find anything to edit to remove it.  I've dug through our pages and tags, but I cannot seem to find what is generating the errant code.
