How to add a multi-select field to the register page in reference checkout?

Does anyone have instructions on how to add a multi-select field to the registration page?   I have need for customers to tell us which sport or multiple sports they are associated with at time of registration?  Netsuite wasn't able to help me.   I can add a single custom field no problem and that is working based on Netsuite answer ID: 41617, but as soon as I make it a "multi-select" it doesn't work.  There must be code to make this happen in the SSP application.  I could use some help for anyone that has this working.  I have modified both the macro/txt file and model.js file to make the custom
