Company Name not displaying on pdf layout forms

Hello,

On all pdf layout forms I have been able to create a workaround by adding a field of text with our company name. The problem is that on the Disclaimer Statement Forms you cannot add custom elements so we are unable to see our companies name on the form. Any help would be greatly appreciated.
