Custom Transaction Form Tabs

Hi,

I am trying to create a custom quote transaction form. I have activated five tabs (Address, Shipping, Communication, Custom and Sales Team, Item is of course on by default) from the tabs tab in the editor. When I save the form and try to use it, only Item, Shipping, Communication and Sales Team shows up. Any idea why the other tabs are now working? I really need the Address tab more than any other except Items.
