Workflow (SuiteScript) Troubleshooting

I have a workflow (SuiteScript) that does not show the 'approve' button to the next approver.  I have been digging into the workflow log, and I think I found the place were the Approve button has been considered but not executed, however I can't figure out why as it meets all the criteria.

I know this is a long shot, but thought if a SuiteScript Guru looked at it, they could point it out in a instant...  and 6+ hours later, I just gave myself a headache...

Any suggestions would be greatly appreciated...

I can see the SuperApprover Button as an Admin; therefore that Add Button is working.
